To Our Patients: We have mailed out letters with a clinical care summary, recommendations regarding how to establish care with another pain clinic, and 2 month’s worth of prescriptions for any patient receiving controlled substances – to bridge to your next pain clinic.
We have established a custodian of records who has agreed to maintain your patient record for the next 7 years. You may request records from the custodian starting in November… our electronic medical records company has informed us that it will take about 1 month to format and transfer patient records.

We are unable to process prior authorization requests, new prescriptions or changes in prescriptions.
We have established a patient assistance phone line on which messages may be left at (520) 339-3147 until November 1. This is not an emergency line, please call your primary care provider, use emergency room services, or call 911 for medical questions if urgent medical problems occur prior to your first visit with your new pain provider.
