		• PATIENT HISTORY: Before your first visit to the Pain Center (I.P.C.A.) you will be asked to fill out a patient history. You will have to do it in our waiting room or you can download an Adobe® Acrobat ".pdf" version of that form.


		• Required Appointment Cancellation: 24 hours. When you arrive for an appointment please be 15 minutes early to complete the paperwork for that day's visit. It is most important to be on time for psychology appointments. They require a fixed amount of time.
